Terms and Conditions
- In order to ensure that the administrative and financial side of your treatment runs as smoothly as possible, you will find the terms and conditions here. When you are treated by the practitioner, a "treatment agreement" is formed. This treatment agreement gives rise to an obligation to pay your practitioner for the services provided.
- The conditions below apply unconditionally to all treatment agreements between patient and practitioner, unless explicitly agreed otherwise in writing. These provisions also apply to follow-up appointments. Dutch law applies to all agreements and assignments.
- Invoicing will, where possible, be made directly to your health insurer immediately after treatment. If this is not possible, you must pay the (remaining) invoice when collecting your podotherapeutic aids, by PIN, iDeal or bank transfer within 14 days. It is not possible to pay your invoice by credit card. When direct billing is not possible, you must arrange the submission of the invoice to your health insurer yourself.
- You are yourself responsible for your entitlement to reimbursement from your health insurer and you cannot derive any rights from the information provided about reimbursements, as I can never be 100% certain that this information is up to date. I naturally try to be as accurate as possible in this regard. I advise you to contact your health insurer for current information.
- In order for the financial side to run smoothly, it is necessary to state your BSN (Burgerservicenummer โ Dutch citizen service number) on the invoice. In medical correspondence, this data is also used to prevent identity confusion. After the first examination, consultation and/or at the end of the treatment, a report will, with your consent, be sent to your doctor or referring practitioner.
- The treatment costs of a patient under 16 years of age are owed by the legal representative. A patient of 16 or 17 years of age is personally liable for the treatment costs, unless the legal representative has confirmed in writing or verbally that they will pay the costs.
- The patient is obliged to reimburse all costs incurred. Verbal consent to the manufacture of podotherapeutic aids such as insoles, orthoses or nail braces is valid. After completing the examination, production of your therapeutic aids is started immediately. In the event of cancellation, all costs incurred will be charged to the patient.
- If the patient defaults on payment, VoorVoet โ praktijk voor podotherapie reserves the right to take collection measures (without further notice). VoorVoet โ praktijk voor podotherapie will engage a debt collection agency and/or a bailiff's office for this purpose. The costs incurred shall be borne by the patient.
- Appointments made, whether verbally or by telephone, can be cancelled or rescheduled up to 24 hours in advance. We reserve the right to charge for appointments that are missed without timely cancellation or that are rescheduled or cancelled within 24 hours of the appointment, unless you can invoke force majeure.
- If you arrive late, this means you will need to make a new appointment. If the schedule allows, we will consider whether you can still be seen. The decision on this rests with your practitioner.
- Podotherapeutic examination follows a referral from a (family) doctor or specialist or after a screening within the framework of Direct Access to Podotherapy. After the examination, you will be informed about the treatment plan and the associated costs. If an aid is manufactured, you will receive the necessary instructions upon delivery. You must at all times contact the treating podotherapist if you experience problems with your podotherapeutic aid, or if you have questions regarding the use and maintenance of the aid.
- Your podotherapist commits to acting to the best of their ability. To safeguard quality and service, the podotherapist is a member of the Nederlandse Vereniging van Podotherapeuten (NVvP โ Dutch Association of Podotherapists) and quality-registered with the Kwaliteitsregister Paramedici (KP).
- Should any complaints arise unexpectedly, please contact your treating podotherapist. You can raise the complaint and a solution will be sought together with you. In certain cases, complaints may be further handled by the complaints committee of the Dutch Association of Podotherapists.
- No guarantee can be given on the effect of therapeutic aids. Insufficient treatment results give no entitlement in any case to a refund or exemption from payment.
- The treatment fees can be found on the website www.voorvoet.nl. VoorVoet โ Praktijk voor podotherapie reserves the right to make changes to the general terms and conditions or fees.
- VoorVoet โ Praktijk voor podotherapie is not liable for the loss, disappearance or damage of your belongings or for any other damage suffered by you.
